It's a mouthful to understand. I don't even know how the government of North Korea works.

According to his Wiki he was "Chairman of the Standing Committee of the Supreme People's Assembly". When you click on that link it says The President of the SPA Presidium is often considered the de facto head of state of North Korea as he is responsible for representing the state and receiving the credentials and letters of recall of foreign diplomatic representatives.

Technically I guess that means the President defected.

I'm still confused as to where the Kims go in their government if they're not prime minister or president.

They recently changed the constitution to make Kim the official head of state, as well as supreme leader. Here's an article reporting that,

I don't know the technicalities, but Iran has a supreme leader also... I think the Kim's are more akin to gods, tho.
