It's dead simple to use. The free version included with Office365 is super, super limited, but still useful for a basic data collection. You can do more conditional stuff with Pro, which is what makes it truly useful.
The whole reason I posted this was because Microsoft doesn't. It's hard to really find what the difference is.

Never even heard of Forms before. Seems kinda neat
