I think it's just human nature/human greed becoming progressively amplified by technology. Someone sees an opportunity to cash in a short-term reward, and the cost/risk gets spread to everyone else.

I feel like we are pushing things faster than we are prepared for. From medicine, to technology and socially, we are pushing things to develop ahead of the organic pace. In some things that may be good, but others it could lead to unforseen consequences.
Let's say we push electric cars in a place that is known for being cold, but the battery technology still doesn't operate in those cold temperatures. Or we push a medical treatment that isn't fully tested and 10 years later it causes major health problems.
I think the modern information age of NOW NOW NOW has leached into things that could have a huge impact later down the road.
