I mine on occasion, did I do it wrong? I don't recall ever having to fill in any info other than what it already had.

oh no, no worries here. This isn't ours. Our stuff has multiple things in place to make sure it all runs right.

This is from the bitcoin main chain. This is a screw-up on a big scale, roughly $160,000.
