Yes, its hard to detect planets that far away, detecting moons is an order of magnitude harder.

Basically: Yes, other planet systems were way too far away to detect moons until recently. Also Yes, most exomoons are likely too small to detect, the one found is planet sized while all other known moons are way smaller.

My best guess is that you're right. The distances are so great and the objects so small.
