Edit: Despite its widespread acceptance as fact, "fake news" appears to be the latest term for yellow journalism, unethical journalism, or tabloid and conspiracy news. It highlights the incestuous nature of news media- journalists reporting on stories from other media organizations- and the general gullibility of people as they get duped by fraudulent media or place too much trust in posts on social media. I don't see evidence of widespread fake news among mainstream credible news organizations.
